Report a concern

Anyone may file — a patient, family member, staff member, or member of the public. You do not need to have read the standard, and you do not need to have raised it with the organization first. Two tracks, because they are answered differently:

Needed for ASF to confirm the outcome to you.

ASF honours this wherever a genuine investigation is possible without disclosure, and will tell you plainly if it cannot be fully preserved because the nature of the concern would identify you regardless.


Urgent
A concern involving a serious adverse event or immediate patient danger is acted on at once, not queued. Say so in the first line.

Policy
Acknowledgement with a case reference and track. Organization response window of four to six weeks. Written confirmation of the outcome. Retaliation against complainants is itself a breach of the standard.
