Theory of Change
Published. Reviewable. Challengeable. No other accreditation body publishes its theory of change. ASF does — because intellectual honesty is not a risk, it is a strength.
1. Transparent measurement creates awareness
When a facility can see precisely where it stands against evidence-based criteria, gaps become specific and actionable. The tool produces a gap report that names each unmet criterion, its clinical significance, and the evidence required to meet it.
2. Awareness combined with professional accountability creates motivation
The Coordinator, Monitor, Director, and Responsible Person are named, certified, and accountable by name. A named, credentialed professional cannot submit a false declaration and disappear. Accountability converts episodic awareness into sustained commitment.
3. Quarterly structure creates habit
Four lightweight cycles per year build quality improvement into operational routine. Improvement becomes continuous rather than performative.
4. Independent verification creates credibility
The Monitor’s verification separates honest self-assessment from optimistic self-assessment. The ADC’s independent decision separates the assessment function from the certification function.
5. Documented trajectory creates evidence
Four quarters of data per facility per year. Published aggregate outcomes annually. Independent academic evaluation of the pilot cohort, submitted to peer review.
6. Local ownership creates replication
Certified Monitors become the distribution network. NAP partners become the local institutional backbone. The system scales without scaling ASF’s cost base.
