Certificate Tiers
Seven tiers of recognition reflecting different stages of the quality improvement journey. Each tier is meaningful and independently verifiable via QR code.
The principle: Recognition should reflect where a facility actually is, not force a binary pass/fail. A facility at “ASF Monitored” has done real, verified work — and its patients deserve to know that, even before full accreditation is achieved.
The Seven Tiers

What Each Badge Can and Cannot Claim

Misrepresentation is a breach. A facility that claims a tier it has not achieved has its registration reviewed and may be removed from the registry. The public registry is the authoritative record — anyone can check it.
Moving Between Tiers
Tiers are progressive. A facility starts at Self-Declared (free), moves to Certified or Registered depending on pathway, progresses to Monitored after the full in-facility verification, and achieves Accredited after the 3-year renewal visit and ADC decision. ISO Ready is an optional add-on for facilities that want to prepare for formal ISO certification. Tiers can also move backward: a lapsed quarterly cycle, a failed re-verification, or a revocation moves the facility down and the registry is updated.
