The Four Roles

The Four Named Roles

Every registered facility names four roles. Each has a defined scope, a defined responsibility, and a clear accountability chain.

Role 1 — Responsible Person

The individual who attests that the self-assessment truthfully reflects the facility’s current state. Typically the CEO, Medical Director, or Owner. Reviews and signs the output. Cannot be the Coordinator or Monitor for the same facility.

“I, [name], [title], [professional registration number], attest that this self-assessment reflects the true current state of [facility name] to the best of my knowledge, and I accept professional and personal accountability for this declaration.”


Role 2 — Director (Delegation of Authority)

Signs the Delegation of Authority before any formal submission. Names all three other roles. Accepts full legal and ethical responsibility for all information submitted to ASF. Must be re-signed annually and within 14 days of any personnel change.

Chain of accountability: Director → Coordinator → Monitor → ADC → Certificate.


Role 3 — Accreditation Coordinator (ASF-Certified)

Manages the accreditation process day to day. Operates the tool. Submits quarterly reports. Does not conduct independent verification.

  • Training: 8 hours per facility type, online, national language
  • Examination: minimum 80%
  • Credential: AC-XXXXX, valid 3 years
  • Cannot be Monitor for the same facility in same or preceding cycle

Role 4 — Accreditation Monitor (ASF-Certified)

Independently verifies the self-assessment against observable evidence. Internal Monitor: one facility only, maximum 3 years. External Monitor: any facility, 2 days/year per facility (Q2 online + Q4 in-facility).

  • Training: 16 hours per facility type, online, national language
  • Supervised observation visit required before certification
  • Examination: minimum 85%
  • Credential: AM-XXXXX, valid 2 years
  • Professional liability clause: false verification = suspension
