The Four Named Roles
Every registered facility names four roles. Each has a defined scope, a defined responsibility, and a clear accountability chain. No anonymous submissions — every declaration is signed by a named, credentialed professional.
Accountability chain: Director & Responsible Person → Coordinator → Monitor → ADC → Certificate. Each layer is independently verifiable. No layer can substitute for another. The Director and Responsible Person may be the same individual, or the Director may delegate attestation to a senior clinical officer who then holds the Responsible Person role.
Role 1 — Responsible Person
The individual who attests that the self-assessment truthfully reflects the facility’s current state. Typically the CEO, Medical Director, or Owner. Reviews and signs the output of every quarterly cycle. Cannot be the Coordinator or Monitor for the same facility.

Role 2 — Director (Delegation of Authority)
Signs the Delegation of Authority before any formal submission. Names all three other roles. Accepts full legal and ethical responsibility for all information submitted to ASF. Must be re-signed annually and within 14 days of any personnel change in the named roles.

Role 3 — Accreditation Coordinator ASF-Certified
Manages the accreditation process day to day. Operates the ASF digital tool. Submits quarterly reports. Maintains the evidence inventory. Does not conduct independent verification — that is the Monitor’s role.

Role 4 — Accreditation Monitor ASF-Certified
Independently verifies the self-assessment against observable evidence. The Monitor’s independence is structural — they are certified by ASF, not by the facility, and their credential can be suspended if they sign off inaccurate verifications.

Professional liability clause: A Monitor who signs off a verification later found to contain significant inaccuracies has their credential suspended and faces formal investigation. This is not policy — it is a contractual obligation signed at certification.
