Thresholds Required

Decision Options
- Accredited: All thresholds met. Certificate within 3 working days.
- Conditionally Accredited: Non-Negotiable met, minor gaps, documented plan. 60-day correction window.
- Deferred: Non-Negotiable gaps. New Q4 + new visit within 6 months.
- Not Accredited: Fundamental failures or evidence of falsification. Full cycle restart.
Annual Renewal
Accreditation is renewed annually. The quarterly cycle continues. The annual supervision visit continues. No separate renewal application — the ongoing cycle is the renewal.
