What it is

A structured fifteen-minute review at the bedside after every fall: circumstances, contributing factors, immediate changes to the care plan, and what the ward learns.

Where to use it

Every fall, within an hour, led by the nurse in charge; the sheet goes to the falls lead and feeds the monthly dashboard.
