Purpose
Records the patient’s wishes for future care, the date, and whether they agreed to share them with the care team.
How to complete
- Hold the conversation and write down the patient’s wishes in their own words.
- Record the date, who was present and any interpreter used.
- Record whether the patient agreed to share the wishes with the team.
