Purpose

Records each anaphylaxis episode and its treatment time, and the daily check of adrenaline and the emergency kit.

How to complete

• Record each daily check of the kit and the adrenaline expiry date.
• Record each episode, the adrenaline given and the time.
• Record the review of each episode.
