Purpose
Records the audit of each set of vital signs, its early-warning score, and the response to any score above the threshold.
How to complete
- Select a sample of observation sets on the schedule set for each ward.
- Check the score was calculated and recorded.
- Where the score was above the threshold, record the response and its time (HOS-SOP-002).
