Purpose

One register for all facility risks: water, fire, equipment, cleaning, signage, power and others.

How to complete

• Add a line for each risk with its source.
• Score likelihood and impact (1 to 5 each, EC).
• Assign one owner, controls, action, due date and review date.
• Update monthly.
