Purpose

Records consent for clinical and before-and-after photographs, and the cooling-off period for elective surgery.

How to complete

• Ask the patient for consent to each use of photographs.
• Record the date of the decision and the cooling-off period.
• Record whether the patient confirmed the decision after the cooling-off period.
