Pain Assessment Scale — VAS/NRS and Reassessment Log
Rate it, treat it, rate it again — pain that is not measured is not managed, and pain that is not reassessed after treatment was never treated
What it is
A one-page pain assessment and reassessment record using the Numeric Rating Scale (NRS 0–10) for adult patients who can self-report, and the behavioural indicators scale for those who cannot. Includes pre-treatment score, intervention recorded, post-treatment reassessment score (within 60 minutes), and escalation trigger (score ≥7 unrelieved). Pain assessment without reassessment after intervention is clinically incomplete and accreditation-ineligible.
