What it is
A weekly grid per toilet block: functioning flush or pit, water available, soap at the hand-washing station, clean (no visible waste), bin emptied, lockable door, light, accessible cubicle, menstrual bin — checked and signed twice daily.
Where to use it
On the inside of every toilet-block door; collected weekly by the cleaning supervisor. Evidence for: Standard 3 — sanitation; Standard 2 — patient environment and dignity.
