What it is
The improvement plan: each gap from the assessment scored for risk (likelihood × harm) and cost (none / low / high), ranked, with the action, owner, date, cost estimate and funding source — plus the quick wins that need no money.

Any facility may translate, add its logo and adapt wording to local practice — keeping the ASF block, the source attribution and the licence line on the sheet.
