What it is
A pre-participation screening form based on the PAR-Q+ general health questions: heart condition, chest pain, dizziness, chronic conditions, medication, bone or joint problems, other reasons — with the decision rule, the signature, and the facility's record.

Reception, before any first session; in the member file; repeated yearly or on any change in health.
