V3.0 — 43 annotated references across 8 evidence domains; evidence methodology statement; ISQua EEA alignment note

1. Purpose

This bibliography constitutes the evidentiary foundation for ASF’s accreditation standards, demonstrating that criteria are grounded in current scientific evidence and international best practice. It is organised into 8 evidence domains and updated at each major standards revision cycle (ASF-GOV-001, §4.1).

2. Evidence Methodology

References were identified through PubMed/Cochrane/Embase systematic searches, hand-searching of systematic reviews, review of published standards from ISQua, JCI, Accreditation Canada, ACHS, and HQACI, and WHO/OECD quality of care frameworks. The bibliography prioritises systematic reviews over single studies; publications from the preceding 10 years; evidence directly linking criteria to patient safety or quality outcomes; and internationally applicable evidence. Where evidence is primarily expert consensus, this is noted and the relevant guidelines are cited.

3. Evidence Domains

Domain 1 — Governance, Leadership and Strategic Management: Evidence linking board quality oversight directly to clinical outcomes (Jha & Epstein, 2010; Dixon-Woods et al., 2011); clinical governance frameworks; WHO governance framework.

Domain 2 — Patient Rights and Ethical Framework: UN CRPD; WHO Declaration on Patient Rights; Beauchamp & Childress four-principle ethics framework; EU cross-border patient rights directive.

Domain 3 — Clinical Care and Patient Safety: WHO patient safety compendium; HAI burden evidence (Allegranzi et al., 2011); WHO Surgical Safety Checklist (Haynes et al., 2009, landmark RCT); medication error evidence (Bates et al., 1995); NPSF safety framework.

Domain 4 — Workforce and Human Resources: Nursing staffing-outcomes evidence (Aiken et al., 2002; 2014, European multi-country study); ICU physician staffing; credential verification standards; WHO HRH 2030 strategy.

Domain 5 — Infection Prevention and Control: WHO IPC core components guidelines (2016); Geneva HHSAF hand hygiene study (Pittet et al., 2000); ECDC IPC framework; multidisciplinary IPC programme evidence.

Domain 6 — Information Management and Medical Records: IOM “To Err is Human” (2000); diagnostic error linked to information gaps; GDPR requirements; WHO medical records manual.

Domain 7 — Physical Environment, Facilities and Equipment: WHO WASH in health facilities; evidence-based healthcare design; NHS healthcare waste management standards; WHO medical devices; IEC electrical safety standards.

Domain 8 — Quality Improvement and Organisational Learning: Berwick (1989) foundational CQI text; Donabedian Structure-Process-Outcome framework; Pronovost Michigan Keystone Project; Greenfield & Braithwaite systematic review of accreditation evidence.
