Purpose
Records the consent conversation for a donation, the person who took it, and the donor’s wishes or the family’s decision.
How to complete
- Record the consent clinician and confirm they are independent of the transplant team.
- Record the donor’s known wishes or the family’s decision.
- Record the time the consent was recorded.
