Purpose
Logs hospital transfers from custody, stopped procedures, safety incidents and each death in custody, with the review made.
How to complete
- Record each hospital transfer with the escort, the times and the handover.
- Record each procedure stopped by health staff and the reason.
- Record each death in custody, the review and the report.
