PDSA Worksheet — Plan, Do, Study, Act
One improvement, one cycle, one page. Small test, quick result, next cycle — the method behind every audit in this library
What it is
The PDSA worksheet: the aim (what, by how much, by when), the measure, the change idea, the prediction, the test plan (who, what, where, when, how small), what happened, what was learned, and the decision — adopt, adapt, abandon — with the next cycle.
Where to use it
Every improvement project; the safety committee expects one per action on the log; three cycles fit on one wall poster. Evidence for: Governance — quality improvement (means of verification: PDSA records). Evidence for: Governance — quality improvement (means of verification: PDSA records).
Translate and adapt
English master edition. Georgian and other translations are added as they are clinically reviewed. Any facility may translate, add its logo and adapt wording to local practice — keeping the ASF block, the source attribution and the licence line on the sheet. Clinical thresholds and drug doses must be confirmed against national protocols before use.
The same code is printed on the tool itself. Scan it in any facility to reach this page.
