Age-Friendly Primary Care — Checklist

What it is

A checklist of what makes a clinic usable and safe for older patients: physical access, information and communication, staff attitude and training, clinical management of the common conditions of ageing, and links to community support.

Where to use it

Clinic manager with a patient from the Patient Council over 65; yearly; gaps into the improvement plan. Evidence for: PHC Standard 2 — access and environment; Standard 3; Standard 4 — care of older people.
