What it is
A monitoring record for procedural sedation: baseline, then every 5 minutes — SpO₂, pulse, BP, respiratory rate, sedation score (Ramsay or similar), oxygen delivered — drug doses with times and cumulative totals, events, and recovery to discharge criteria (Tool 4).
Where to use it
