Purpose
The survey given to patients each year, to record their experience of care and what they would change. It is returned privately.
How to complete
- Give the form to each patient in the sample, and help them complete it if they need help.
- Keep the forms in the sealed box until the survey closes, then count the responses.
- Record the response rate and the main findings in the annual summary.
