Purpose
Records the nurse-to-patient ratio on each shift against the ratio for the acuity, and any shortfall and its escalation.
How to complete
- Record the ratio on each shift, including nights and weekends.
- Record the acuity mix on the shift.
- Record any shortfall, its escalation and the action taken.
