Purpose
Records each verification of cytotoxic handling practice: equipment worn, spill kit present, and waste disposed of correctly.
How to complete
- Observe a sample of preparations, administrations and disposals each month.
- Record whether the right protective equipment was worn.
- Record the spill kit check and the waste disposal.
