Credential Verification Checklist — Before the First Shift
Identity, qualification, licence, references, health, record check: verified at source, not from a photocopy, and signed off before anyone touches a patient
What it is
A pre-employment verification checklist for clinical staff: identity documents, qualification verified with the awarding institution, professional licence verified with the regulator (number, expiry, conditions), two references from the last employers, employment-gap explanation, criminal-record check where lawful, occupational health and vaccination, right to work, signed confidentiality and code of conduct — with the sign-off and the file index.
Where to use it
HR and the medical director for every clinical appointment, including locums and volunteers; the file audited yearly. Evidence for: Governance — human resources; Policy 8 (Staff Credentialing); Standard 4 — safe care by qualified staff. Translate and adapt
English master edition. Georgian and other translations are added as they are clinically reviewed. Any facility may translate, add its logo and adapt wording to local practice — keeping the ASF block, the source attribution and the licence line on the sheet. Clinical thresholds and drug doses must be confirmed against national protocols before use.
The same code is printed on the tool itself. Scan it in any facility to reach this page.
