142 patient booklets. A4, folded in three, free. Keep the ASF mark.
Procedures, conditions, diagnostics, home care, rights — written once, clinically reviewed, printed anywhere
Why they exist
Patients forget most of what they are told in a consultation, and the internet tells them the rest, unreliably. These 142 booklets exist so that a Georgian clinic can hand every patient one page of accurate, plain-language information on the procedure they are having, the condition they are managing, or the right they are exercising — written and clinically reviewed once, printed anywhere. They were developed by ASF with the Public Health Institute of Georgia and David Tvildiani Medical University because the facilities in the accreditation programme asked for them: patient information is required by the standards, and producing it clinic by clinic is beyond most budgets.
Each booklet is an A4 sheet folded in three, designed to print on an office printer and to be read in five minutes. Each carries a QR code that opens the booklet’s own page here, so a patient who lost the paper can find it again, and a clinic manager who scans one booklet finds the whole library. The set is organised in 24 categories, from diagnostics and medical procedures to chronic disease, home care and patient rights — with Patient Rights in five languages. Everything is free for any facility in any country to print, distribute and translate; the only condition is that the ASF block stays on the sheet.
Terms and adaptation
Print, distribute on paper or electronically, translate into any language, add your own logo and contact details. Keep the ASF block and QR code. Do not alter the clinical content without clinical review; tell us what you changed.
Each booklet has a panel for the clinic’s own logo, name and contact line. The guides below show how; the editable template is available on request. Evidence for ASF Standard 2 (Reception & Information) and Standard 6 (Aftercare).
