Why they exist

The best patient-safety tools in the world are free, and most facilities have never seen them. The WHO, IAEA, AHRQ, CDC and NHS publish checklists, audit forms, self-assessments and templates under licences that allow anyone to adapt and translate them — but they sit in long English PDFs with no layout a ward can print. ASF takes each one, rebuilds it as one A4 sheet in the house style, states the source and licence in the footer, maps it to the ASF criterion it evidences, and gives it its own page and QR code — so a facility working through its self-assessment sees “criterion 4.6 → here is the form”.

The library is organised in six tiers and thirty-two numbered toolkits. Every tool carries a reference code — ASF-TOOL-[toolkit]-[number] — and is numbered within its toolkit (for example 1.3 is the third tool of Toolkit 1). Each toolkit can be downloaded as one zip.

Terms and clinical review

Licences, stated on every sheet

WHO and IAEA material is used under CC BY-NC-SA 3.0 IGO; US federal material (AHRQ, CDC) is public domain; NHS material under the Open Government Licence v3.0; PAR-Q+ with attribution as its terms permit. ASF originals and ASF’s additions — layout, guidance, evidence mapping — are released under CC BY-NC-SA 4.0. Each sheet names its source and licence in the footer.

English masters — clinical review before use

These are English master editions. Drug doses, clinical thresholds, colour schemes and legal references must be confirmed against national protocols and law by the facility’s clinical and legal leads before use; the sheets say where. Georgian editions follow clinical review. Nothing here is required by ASF — the standards describe outcomes, not forms — but nothing here needs to be invented twice.
